If the property is being sold, the current owner must be listed, and we can update it later with the deed of sale.<\/span><\/p>\n<p align=\"left\"><a href=\"https:\/\/accessibilityunlimited.com\/wp-content\/uploads\/2025\/09\/OAD.pdf\"><strong>Owner Agent Designation Form<\/strong><\/a><span style=\"font-family: Arial;\"><br \/>\nDesign Professionals, or any other entity that is a third party to the owner, must submit this form if they have been assigned by the Owner to be their Designated Agent.<br \/>\n<\/span><\/p>\n<p align=\"left\"><strong><a href=\"https:\/\/accessibilityunlimited.com\/wp-content\/uploads\/2025\/09\/LLO.pdf\">Limited Liability Form<\/a><\/strong><br \/>\nThe new Limited Liability Owner (LLO) form provides needed documentation for owning entities that are created but have no representatives on staff to sign documents. The new LLO form will be required for all owner entities that have a parent or umbrella entity signing on the owner entity\u2019s behalf. <span style=\"text-decoration: underline;\">A copy of the Articles of Formation (AOF) or copy of the Texas Secretary of State (SOS) document is also required.<\/span><br \/>\nWe also marked up a copy of the LLO form with tips on filling it out correctly, <strong><a href=\"https:\/\/accessibilityunlimited.com\/wp-content\/uploads\/2025\/10\/LLO-with-tips.pdf\">here<\/a><\/strong>. Please use this version for reference only.<\/p>\n<p><a href=\"https:\/\/accessibilityunlimited.com\/wp-content\/uploads\/2023\/09\/POS.pdf\"><strong>Proof of Submission Form<\/strong><\/a><br \/>\n<span style=\"font-family: Arial;\">An architect, interior designer, landscape architect, or engineer with overall responsibility for the design of a building or facility shall mail, ship or hand-deliver the construction documents along with a Proof of Submission form to a Registered Accessibility Specialist (RAS) not later than the twentieth (20) business day after the plans and specifications have been issued.<br \/>\n<\/span>Issue &#8211; To mail, deliver, transmit, or otherwise release plans or specifications to an owner, lessee, contractor, subcontractor, or any other person acting for an owner or lessee for the purpose of construction, applying for a building permit, or obtaining regulatory approval after such plans have been sealed by an architect, interior designer, landscape architect, or engineer.
