Registration Form
To obtain a TABS number, complete the registration form and submit along with one set of plans and plan review fee to a Registered Accessibility Specialist (RAS).
The design professional may also register the project online at the TDLR website.  Once registered, submit the confirmation page to a RAS along with one set of plans, fee, and Proof of Submission form.  If the design professional is registering the project, the TDLR filing fee must be paid prior to receiving the TABS number.  The filing fee is paid during the registration process on the TDLR website.

Owner Agent Designation Form
Design Professionals must submit this form if they have been assigned by the Owner to be their Designated Agent.  Please use this form, as the old version without the CAD number field will no longer be accepted.

Proof of Submission Form
An architect, interior designer, landscape architect, or engineer with overall responsibility for the design of a building or facility shall mail, ship or hand-deliver the construction documents along with a Proof of Submission form to a Registered Accessibility Specialist (RAS) not later than the twentieth (20) business day after the plans and specifications have been issued.
Issue – To mail, deliver, transmit, or otherwise release plans or specifications to an owner, lessee, contractor, subcontractor, or any other person acting for an owner or lessee for the purpose of construction, applying for a building permit, or obtaining regulatory approval after such plans have been sealed by an architect, interior designer, landscape architect, or engineer. In the case of a state-funded or other public works project, it is the time at which plans or specifications are publicly posted for bids, after such plans or specifications have been sealed by an architect, interior designer, landscape architect, or engineer.

Request for Inspection form
Use this form to request an inspection on a project that has been completed.  This form is required to be submitted by the Owner or Designated Agent to a Registered Accessibility Specialist before an inspection can be performed.  The inspection must be requested no more than 30 days after the project completion date.

Inspection Response Form (Peter Grojean / Jessica Hamm)
Building/facility owners or their agents may use this form to indicate the status of outstanding violations cited as a result of a Texas Accessibility Standards (TAS) inspection.  To ensure proper processing, follow each step making sure to return the completed form (fax, mail, or hand-deliver) to the Registered Accessibility Specialist (RAS) that inspected the building/facility.
